Booking & Patient Consent

Last updated: August 2026

By booking and attending an appointment with T-Leaf Wellness, you agree to the following Booking & Patient Consent terms.

1. Making an Appointment

Appointments with T-Leaf Wellness are provided by telehealth unless otherwise advised.

When making a booking, you agree to:

  • provide accurate and complete personal and health information;

  • select the appropriate appointment type;

  • provide valid contact and payment details;

  • be available at your scheduled appointment time; and

  • provide any information reasonably requested before your consultation.

If the appointment type selected does not correspond with the consultation required, the appropriate consultation fee may apply.

2. Consultation Fees

You agree to pay the fee displayed for your appointment at the time of booking.

Unless expressly stated otherwise, consultation fees do not include the cost of medications, pharmacy dispensing, pathology, imaging or other third-party healthcare services.

Cancellations, rescheduling and missed appointments are managed in accordance with our Cancellation Policy.

3. Telehealth Appointments

You are responsible for ensuring that you have a suitable telephone, internet connection or device for your consultation.

Where possible, please attend your appointment from a private and quiet location where you can speak openly with your practitioner.

Occasional delays may occur because healthcare consultations can take longer than anticipated.

If technical difficulties prevent a consultation from proceeding, T-Leaf Wellness will make reasonable efforts to reconnect or arrange an appropriate alternative.

4. Your Health Information

To provide safe and appropriate care, your practitioner needs accurate information about your health.

You agree to disclose relevant information including:

  • your medical and mental health history;

  • current and previous medications and treatments;

  • allergies and adverse reactions;

  • alcohol, tobacco and other drug use where clinically relevant;

  • pregnancy, breastfeeding or plans for pregnancy where relevant; and

  • any significant changes in your health.

You should tell your practitioner if any information you have previously provided is no longer accurate.

5. Consent to Healthcare

By proceeding with your consultation, you consent to being assessed and receiving healthcare advice through telehealth.

You understand that:

  • treatment recommendations are made following an individual clinical assessment;

  • not every treatment is appropriate for every patient;

  • no particular medication, prescription or treatment is guaranteed;

  • your practitioner may recommend further investigation, follow-up, referral or an alternative treatment;

  • all treatments and medications can have potential benefits, risks and side effects; and

  • treatment may not produce the outcome you expect.

Your practitioner will discuss clinically relevant treatment options, risks and precautions with you before proceeding where appropriate.

6. Prescribing and Medication

Where medication is being considered, prescribing remains entirely at the clinical discretion of your treating practitioner.

A consultation does not guarantee that a prescription will be issued.

If medication is prescribed, you agree to:

  • use it only as directed;

  • not alter your dose without appropriate clinical advice;

  • not share prescribed medication with another person;

  • inform your practitioner of significant side effects or adverse reactions; and

  • attend follow-up appointments where clinically required.

Your practitioner may change or discontinue treatment where they consider this clinically appropriate.

7. Driving and Safety

Some medications can cause sedation, impaired concentration, altered reaction time or other effects that may make driving or operating machinery unsafe.

You must follow the advice provided by your practitioner and pharmacist regarding driving, operating machinery, alcohol, other medications and activities requiring alertness.

You remain responsible for complying with applicable Australian road and safety laws.

8. Health Summary and Other Healthcare Providers

Where reasonably required for your care, we may ask you to provide a current health summary or other relevant medical information.

With your consent, T-Leaf Wellness or your treating practitioner may communicate with your GP, pharmacist or other healthcare providers where appropriate for the safe coordination of your care.

Your health information will be handled in accordance with our Privacy Policy and applicable Australian privacy and health-records legislation.

9. Pharmacy Choice

If a prescription is issued, it may be sent to a pharmacy able to facilitate dispensing and delivery.

You may nominate your preferred pharmacy, subject to applicable prescribing, dispensing and administrative requirements.

Availability, dispensing, pricing and delivery of medication are ultimately subject to the relevant pharmacy and applicable Australian laws and regulations.

10. Follow-Up Care

Your practitioner may recommend or require follow-up consultations to review your response to treatment, assess safety or determine whether continued treatment remains clinically appropriate.

You are responsible for attending recommended follow-up appointments and advising us if your health circumstances change.

11. Recording Consultations

To protect patient and practitioner privacy, consultations must not be recorded, photographed or filmed without the prior consent of everyone participating.

Your practitioner may end a consultation if an unauthorised recording is taking place.

12. Respectful Conduct

T-Leaf Wellness is committed to providing a safe and respectful environment for patients, practitioners and staff.

We ask that all communication remains respectful and appropriate. Threatening, abusive, discriminatory or seriously disruptive behaviour may result in a consultation being ended or, where appropriate, discontinuation of services.

Where ongoing care is discontinued, reasonable steps will be taken to support continuity of care where clinically appropriate.

13. Emergencies

T-Leaf Wellness is not an emergency medical service.

If you are experiencing a medical emergency or require urgent medical attention, call 000 or attend your nearest emergency department.

14. Your Consent

By booking and proceeding with your consultation, you confirm that:

  • I have provided accurate and relevant information about my health and medications.

  • I understand that booking a consultation does not guarantee a prescription or any particular treatment.

  • I understand that treatments and medications may have benefits, risks and side effects and that these will be considered as part of my individual clinical assessment.

  • I consent to receiving healthcare via telehealth and to my information being collected, used and disclosed as reasonably necessary for my care and in accordance with the T-Leaf Wellness Privacy Policy.

  • I have had the opportunity to ask questions and understand that I may withdraw my consent to treatment at any time.
